CONFESSION
Pray to the Holy Spirit for self-knowledge and trust in the mercy of God. Examine your conscience, be truly sorry for your sins, and resolve to change your life.
Go to the priest and begin with the Sign of the Cross. Welcoming you, the priest will say:"May God, who has enlightened every heart, help you to know
your sins and trust in his mercy," or similar words taken from Scripture. You answer:"Amen. Then say, "Bless me, Father, for I have sinned. It has been
____ weeks/ months/years since my last confession."
Confess your sins openly and candidly. Tell the priest of all mortal sins and the number of times each was committed, and then you may confess some of your venial sins. (Although it is not strictly necessary to confess venial sins, the Church recommends that you do.) If you do not know whether a sin is mortal or venial, ask the priest. If you have no mortal sins, confess venial sins you have committed since your last confession; you may also mention some mortal sin from your past life for which you are particularly sorry, indicating that it has already been confessed.
When you are done listing your sins please say:
"I am sorry for these sins and all the sins I can’t remember.”
Then listen to the priest for whatever counsel he may judge appropriate. If you have any question about the faith, how to grow in holiness, or whether something is a sin, feel free to ask him. Listen to the advice the priest gives you and accept the penance from him. Then make an Act of Contrition for your sins:
You may say for example: "Lord Jesus, Son of God, have mercy on me, a sinner."
or an "Act of Contrition"
My God,
I am sorry for my sins with all my heart.
In choosing to do wrong
And failing to do good,
I have sinned against you
whom I should love above all things.
I firmly intend, with your help,
to do penance,
to sin no more,
and to avoid whatever leads me to sin.
Amen.
The priest will then conclude with the words: “Your sins are truly forgiven, Go in Peace”. You respond: “Thanks be to God.”
Do the penance the priest assigns you.
If you are anxious or unsure of what to do, take this guide with you or tell the priest and he will make it easier for you.

Spotkałam się z kilkoma tekstami Act of Contrition.
Zamieszczam je wszystkie poniżej.
Act of Contrition
O my God, I am heartily sorry for having offended Thee, and I detest all my
sins because of Thy just punishments, but most of all because they offend
Thee, my God, Who art all-good and deserving of all my love. I firmly
resolve, with the help of Thy grace, to sin no more and to avoid the near
occasions of sin.
Act of Contrition
My God, I am sorry for my sins with all my heart. In choosing to do wrong
and failing to do good, I have sinned against you whom I should love above
all things. I firmly intend, with your help, to do penance, to sin no more,
and to avoid whatever leads me to sin. Our Savior Jesus Christ suffered and
died for us. In his name, my God, have mercy.
Act of Contrition
O my God, I am sorry for my sins because I have offended you. I know I
should love you above all things. Help me to do penance, to do better, and
to avoid anything that might lead me to sin. Amen.
Listen as the priest absolves you of your sins and enjoy the fact that God has truly freed you from all your sins. If you forget to confess a mortal sin, you
are still forgiven, but must mention it the next time you go to confession.
Act of Contrition
"O my God, I am heartily sorry for having offended Thee, and I detest all my sins, because I dread the loss of Heaven, and the pains of Hell; but most of all because I love Thee, my God, Who art all good and deserving of all my love. I firmly resolve, with the help of Thy grace, to confess my sins, to do penance, and to amend my life. Amen."
